I've spent maybe five minutes in a chat room. I was a latecomer to the Internet, mainly because I was struggling to make ends meet during the '90s and couldn't find the time or the money to be wired. Since then, however, I've been a regular blog lurker, and I've noticed that most blog comment threads - but not all - are not helpful, and for all the reasons that you've observed.

Jeff, you have pretty much nailed the usual comment thread paradigm. (Except for Josh's blog, of course!!)

Far too many forums/discussion threads/etc., seem to follow this format:

1) Point A
2) Point B
3) Point C
4) Point A/B/C is wrong because ______.
5) "Your rebuttal of my point is wrong because you're a moron."
6) "Well, you're a Nazi."
7) "Can't we all just get along?"

Not the most productive way to spend one's time, indeed. Reasoned, civil discourse is certainly something that is absent from our society.
